How do I get my form online?
The 2022 forms will be available online through Doculivery at http://www.doculivery.com/unitedfamilypayroll.
The only way to get your 2022 Form W-2 online is by logging into your Doculivery user account and opting in.

Can I have my Form W-2 e-mailed to me?
No, Form W-2s will not be sent electronically. The form can only be accessed through your Doculivery user account.
What if I no longer work for The United Family?
Even if you are no longer actively working, you can still opt in to receive your 2022 Form W-2 online. Click here for the instruction guide.
When will my 2022 Form W-2 be available online?
2022 W-2 forms will be available no later than January 31, 2023. An e-mail will be sent to your e-mail address on record to notify you when the form is available.
